How to apply for Life Certificate

The Life Certificate is issued directly by the High Commission. The issue of ‘Life Certificate’ has NOT been outsourced to BLS International Services Ltd. or any other agency.

The Pensioner wishes to obtain a Life Certificate from the High Commission of India, Ottawa is required to visit High Commission (10 Springfield Road, Ottawa) after seeking prior appointment.

STEP 1- Arrange the documents for Life Certificate:

1. Application form duly filled in. The form is required to be singed in the High commission only. The Form may be downloaded from the following link:

2. Passport
3. PR Card/Visa (for Indian Passport holder)
4. Pension Payment Order (PPO)
5. OCI Card (If available)
6. Address Proof (Driving Licence, Health Card, Utility Bill - wifi, mobile, hydro, etc)

One set of the Photocopy of documents with original is required to be submitted at the counter.

STEP – 2:  Appointment for Life Certificate:

Once you are able to arrange the documents, you can book appointment on Suvidha Portal (at the below link) for visiting High Commission of India, Ottawa, (10 Springfield Road, Ottawa, Ontario, K1M 1C9] to obtain Life Certificate :

Note:

1. One set of the Photocopy of documents with original is required to be submitted at the counter.
